Office manager always busy scheduling appointments for medical specialist for both patients and theatre. Developed an information sheet using Microsoft Word for new patients to complete so that the information can be added to their database.
Word document was exceedingly difficult to use for the end user as the patients' struggled to provide the information in the space provided. Field length was inconsistent - space for 'post code' and 'suburb' too large. Medicare information was not prominent and some information requested twice - once on the front page and then again on the reverse.
Strip the content from the Word document and provide a new layout where the information required is categorised according to type. Colour added to segregate areas with boxes to show where information is required.
Final document provided as PDF to enable local printing as required.
